Find a MultiPlan Provider

You can find providers and facilities on the MultiPlan website. Follow this step-by-step navigation or view the video below to guide you.

1. Click the MultiPlan Provider Directory button below. It will open in a separate window.
2. Under Choose a Network, select "Practitioner & Ancillary"
3. Enter your facility/practitioner and zip code
